According to miwam.uis.edu, the MI Web Account Manager (MiWAM) is “the UI’s system for filing and claim and managing your own UI account online at miwam.uis.edu."

From the miwam homepage, click on File a new claim under Login in the top right corner.

Select how you would like to file your claim - whether it is initial, reopen or continued claim.

After clicking Continue, select your reason for filing from the list below and then click Continue .

From the Provider selection page, you must choose Other options at the bottom of the providers list. If there is no option to choose Other , then that means that MiWAM does not support filing claims on those services quite yet. You can either continue with your claim and send an email later with your provider information or go ahead and choose your provider.

After selecting Other , select the service type you are filing for from the list below and then click Continue .

Under Provider's Name , enter in NPI number. The NPI number should be under 10 digits. If it is more than 10 digits, it will not work.

After entering the NPI number , click Apply . You should see your name listed as your provider on the top of the list and a three-digit code next to it.

This plan can be purchased if you do not have coverage from another insurance carrier or Medicare Part B . If purchased through this website, temporary cards will be mailed via USPS to the address listed under the Beneficiary tab. If you are using ExpressSOS, your MiWAM card will be sent directly to the provider office.

After clicking Continue , enter in your date of birth . You must be at least 16 years old to file for this service plan. The next screen you see should have your name , date of birth and sex already filled out with information that was previously recorded on the system. Please verify this information is correct before continuing on. If it is incorrect, click Edit . If everything is correct, click Save changes and continue .

After clicking Save changes and continue , click Continue . You will now be asked to create a password and verify the password you just created. Please do not lose this information as it is the only way you can access your account. After typing in your password, please retype it in the box below to confirm that it is correct. Click Continue after verifying that both passwords match up.
